EMA Backs Lilly’s Foundayo Pill for Weight and Diabetes

Eli Lilly’s Foundayo won EMA backing as a no-fasting obesity and diabetes pill, with Brussels and national payers still to act.

The European Medicines Agency’s human medicines committee recommended approval of Eli Lilly’s Foundayo pill on 15 October 2026 for adult weight management and type 2 diabetes. The once-daily tablet, whose active ingredient is orforglipron, still needs a European Commission decision before it can be sold in the EU.

Novo Nordisk’s Wegovy pill already has that Commission stamp, from 15 July 2026, and showed a larger average weight loss in its own trial. Lilly is offering a different profile: a small non-peptide molecule that can be taken at any time of day, with or without food, and a diabetes claim the Wegovy tablet does not carry in Europe.

Europe’s Second Oral GLP-1 Gets a Dual Mandate

The Committee for Medicinal Products for Human Use, known as the CHMP, adopted a positive opinion at its 12 to 15 October 2026 meeting. On 16 October the agency recommended granting a marketing authorisation for Foundayo as a prescription tablet for both uses in adults.

The EU applicant is Eli Lilly Nederland B.V. Foundayo will be sold as 0.8 mg to 17.2 mg film-coated tablets in six strengths: 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg and 17.2 mg. The active substance is listed as orforglipron hemicalcium.

For weight, the opinion covers adults with a body mass index of at least 30, or at least 27 with a weight-related condition such as prediabetes, type 2 diabetes, high blood pressure, abnormal blood fats, obstructive sleep apnoea or cardiovascular disease, used with a healthy diet and physical activity. For diabetes, it covers adults whose blood glucose is not well controlled, used alone when metformin is not appropriate, or with other diabetes medicines.

The agency framed the product as a once-daily alternative to injectable GLP-1 medicines and to other oral ones. Treatment starts at a low dose and rises with response and tolerability. The most common side effects were nausea, vomiting and diarrhoea, which showed up more often while the dose was going up and eased over time.

That dual label is the part of the file Novo’s obesity tablet does not match in the EU. The Wegovy pill is cleared there for weight management. Novo’s older oral semaglutide brand, Rybelsus, is the diabetes tablet. Foundayo would put both jobs on one bottle if the Commission follows the committee.

Foundayo Can Be Taken With Breakfast

Orforglipron is a small, non-peptide molecule that turns on the GLP-1 receptor. Peptide pills such as oral semaglutide have to survive the gut, which is why they come with a fasting script. The agency says Foundayo can be taken at any time of day, with or without food, because it was built to be swallowed as a conventional tablet.

The Wegovy pill is oral semaglutide 25 mg. EMA instructions for that tablet tell people to take it on an empty stomach after at least eight hours of fasting, then wait 30 minutes before eating, drinking or taking other medicines. Miss the window and absorption can suffer. Foundayo’s label, as the committee described it, does not ask for that clock.

HOW THE TWO PILLS ARE TAKEN

  • Foundayo timing: Once a day, at any hour, with or without food, and with no water limit described by the agency.
  • Wegovy pill timing: Once a day on an empty stomach after at least eight hours of fasting, then a 30-minute wait before food, drink or other oral medicines.
  • Foundayo titration: Start low and raise the dose with response and how well the patient tolerates it, using the six tablet strengths.
  • Access: Both would be prescription-only in the EU; Foundayo is not authorised for sale there until the Commission acts.

That gap will matter in clinics more than it matters on a slide. A tablet that can sit next to coffee is easier to stay on than a tablet that needs an overnight fast and a clear half hour after the dose. It is also the feature Lilly has pushed since the US launch. The open question is whether European doctors will trade a simpler routine for a smaller average loss on the scale.

What the Diabetes Studies Showed

The committee reviewed four main type 2 diabetes studies in 3,765 adults whose blood glucose was not adequately controlled with diet and activity, or with other diabetes drugs. Foundayo was used alone or with other medicines and was compared with placebo, dapagliflozin 10 mg, or oral semaglutide at 7 mg and 14 mg. The main measure was the change in HbA1c, the standard marker of long-term glucose control.

Across those studies, Foundayo cut HbA1c by around 1.2 to 1.9 percentage points, depending on the dose. Placebo groups saw cuts of 0.4 to 0.8 percentage points. Dapagliflozin 10 mg cut HbA1c by 0.8 percentage points. Oral semaglutide cut it by 1.2 to 1.5 percentage points at 7 mg and 14 mg. The ranges overlap. The file does not show a clean win on glucose against Novo’s diabetes tablet at those doses.

The agency also noted the public-health backdrop. Obesity and overweight affect more than 2.5 billion people, a figure it tied to the WHO’s account of adult overweight and obesity worldwide. In 2021, excess body weight was linked with around 4 million deaths and 129 million disability-adjusted life years. Diabetes affects about 589 million adults aged 20 to 79, with type 2 making up about 90% of cases.

For health systems, a single oral GLP-1 that can be written for weight or for glucose is simpler paperwork than two brands. It is also a new budget line. Diabetes drugs are often reimbursed on different terms than obesity drugs, and member states already ration weekly shots. A dual-label pill does not magically land on the diabetes tariff.

The Weight-Loss Gap With Oral Wegovy

EMA’s weight recommendation rests on two studies in 4,740 adults with obesity or overweight. One enrolled people without type 2 diabetes who had another weight-related problem. The other enrolled people with type 2 diabetes. After 72 weeks, people on Foundayo lost around 5% to 11% of body weight, depending on dose, compared with around 2% to 2.5% on placebo.

Novo’s EU case for the Wegovy pill used the OASIS programme, including OASIS 4, a 64-week trial of once-daily oral semaglutide 25 mg in 307 adults. Novo said people on that pill lost about 17% of body weight versus about 3% on placebo when they stayed on treatment, and that about one in three lost 20% or more. Side-effect dropouts were 6.9% on the pill and 5.9% on placebo.

Those are not head-to-head numbers. The trials ran for different lengths, in different groups, and Novo’s 17% figure is the effect if people adhered. Even with that caveat, Foundayo’s agency-reported average sits well below the figure Novo took to Brussels. Weekly shots already on European labels go further still. The convenience case has to carry a product that loses less weight on paper.

Measure Foundayo (orforglipron) Wegovy pill (semaglutide 25 mg)
EU status CHMP positive opinion, 15 October 2026; Commission pending Commission approval, 15 July 2026
EU indications in this file Weight management and type 2 diabetes Weight management
Molecule Small non-peptide GLP-1 agonist Peptide GLP-1 agonist
Food and water Any time, with or without food Empty stomach after ≥8 hours fasting; 30-minute wait
Weight-loss figure cited for EU Around 5% to 11% at 72 weeks vs 2% to 2.5% placebo About 17% vs 3% placebo in OASIS 4 (64 weeks, if adhered)

Payers who already balk at covering shots for BMI alone will read that table as a reason to wait, or to steer patients toward the Novo tablet where it is listed. Doctors who have watched patients quit injections may read the same table as a reason to start with the pill that does not need a fast. Both readings can be true in the same country.

Chugai Licensed the Molecule for $50 Million

Orforglipron did not start in Indianapolis. On 27 September 2018, Chugai Pharmaceutical Co., Ltd. and Lilly announced a license for OWL833, then a phase 1-ready oral non-peptide GLP-1 agonist aimed at type 2 diabetes. Lilly took worldwide development and commercialization rights. Chugai received $50 million upfront and stays in line for milestones and royalties if the product sells.

We believe OWL833 can be a best-in-class oral non-peptide GLP-1 receptor agonist and that its value will be further enhanced through Lilly’s clinical development to contribute to people around the world who live with diabetes.

Yasushi Ito, Executive Vice President, Chugai Pharmaceutical, 27 September 2018 license announcement

Daniel Skovronsky, then Lilly’s chief scientific officer, called the asset a step toward the next generation of diabetes therapies. Eight years later the same molecule is a weight-loss brand with a US launch, a UK launch, and a European committee opinion. Chugai’s royalty stream is the quiet line in Lilly’s Foundayo P&L, and it grows if Europe actually reimburses the pill.

THE ORAL GLP-1 PATH TO EUROPE

  1. 27 September 2018: Lilly licenses OWL833 from Chugai for $50 million upfront plus milestones and royalties.
  2. 1 April 2026: The US Food and Drug Administration approves Foundayo for adult weight management.
  3. 22 May 2026: CHMP recommends adding Wegovy tablets for adult weight management.
  4. 15 July 2026: The European Commission authorises the Wegovy pill as the first oral GLP-1 for weight management in the EU.
  5. August 2026: Foundayo launches in the UK.
  6. 15 October 2026: CHMP adopts a positive opinion on Foundayo for weight management and type 2 diabetes.

Small-molecule chemistry is also a factory story. Peptide injectables need specialised fill-finish lines and, often, a cold chain. A film-coated tablet can run on ordinary oral-solid plants. Lilly held a groundbreaking on 21 September 2026 for a manufacturing campus at Generation Park in Houston. Europe’s supply question is whether those plants, and others already making the US and UK packs, can feed 27 markets if payers say yes at once.

Member States Will Set Price After Brussels

The CHMP opinion is an intermediate step. It goes to the European Commission for an EU-wide marketing authorisation. The agency does not set a clock in its notice. After a Commission yes, price and reimbursement are decided in each member state, against that country’s own health system and against the other GLP-1s already on contract.

Novo is already on that path. The Commission’s 15 July 2026 decision made the Wegovy pill the first oral GLP-1 for weight management across EU member states, Novo’s fifth clearance for that tablet after the United States, the UK, the UAE and Bahrain. Novo said it planned more launches in the second half of 2026. Mike Doustdar, Novo’s president and chief executive, tied the tablet to people who will not start an injection.

Obesity is a serious chronic disease, and choice can make a real difference. For many people, a tablet may be a simpler and more acceptable way to start and continue treatment.

Mike Doustdar, President and CEO, Novo Nordisk, 15 July 2026 Commission approval statement

Foundayo’s simpler swallow routine undercuts part of that pitch. Novo still has the first-mover listing, the 17% trial figure, and a cardiovascular story it advertises for the Wegovy franchise. National agencies that cap obesity spend will ask Lilly to justify a second oral GLP-1 that loses less weight. Diabetes departments may ask a different question: can this tablet replace a weekly shot in people who refuse needles and cannot follow Novo’s fasting rules.

WHAT WE KNOW

  • CHMP opinion: Positive on 15 October 2026 for adult weight management and type 2 diabetes.
  • Next legal step: The European Commission decides on an EU-wide marketing authorisation.
  • Money step: Price and reimbursement are set by each member state after that decision.

WHAT IS UNCONFIRMED

  • Commission date: No decision day is set in the agency notice.
  • Launch wave: Lilly has not posted an EU ship date tied to this opinion.
  • National lists: No member state has priced Foundayo, because it is not yet authorised.

Until those three blanks fill in, a positive CHMP vote does not put a pack on a German or French pharmacy shelf. It puts a dossier on a Commission desk, then on 27 pricing desks that have already been burned by GLP-1 demand.

$98 Million From the First Sales Quarter

Foundayo reached US pharmacies after the April 2026 approval and reached the UK in August 2026. Lilly booked $98 million in sales in the second quarter, the first period with a US launch on the books. The company has sought approval in more than 40 countries. Early US use has been read as proof that some patients who will not inject will swallow a daily pill.

FOUNDAYO OUTSIDE THE EU

  • US launch: April 2026, after FDA approval on 1 April 2026 for adult weight management.
  • UK launch: August 2026.
  • Q2 sales: $98 million.
  • Filings: More than 40 countries.

That run-rate is a start, not a European forecast. The obesity-drug market is widely expected to surpass $100 billion by 2030, and oral products are the next share fight inside it. Lilly is due to report third-quarter 2026 results on 29 October 2026, which will be the first look at Foundayo sales after a full US summer. None of those figures tell a ministry what it will pay in 2027.

The committee gave Lilly a scientific yes for a tablet that is easier to take than Novo’s and broader on paper because it includes type 2 diabetes. Novo still has the EU license, the larger trial loss, and months of listing time. The next move is the Commission’s, then the payers’.
